アルコール使用障害と併存症

アルコール使用障害(AUD)と併存症についてまとめてみた。

アルコール使用障害の概観

  • アルコール使用障害(AUD)は、医療費の4%を占める慢性疾患および再発性疾患である(Margolese et al., 2004 ; Rehm et al., 2009)。
  • 12ヶ月と生涯AUD(DSM-5)の有病率はそれぞれ13.9%と29.1%であると報告されている(Grant et al.,2016)。
  • AUDは、傷害、疾病、死亡(Barbosa et al.,2010 ; Dawson and Grant, 2011)、および相当な財務コスト(Joshua, 2017)を含む多くの有害な結果のリスクを増加させる。
  • 初期の生活ストレスやストレスの多い生活状況(例えば、愛する人の死、離婚)を含む不利な生活環境(Holgate and Bartlett, 2015)。
  • ストレスと同様に、生活習慣の満足度はアルコール使用および飲酒に関連している(Peltzer and Pengpid,2016)。
  • 精神障害を有する個人は、しばしばAUDとの併存疾患を示す(Sher,2006)。
  • 統合失調症気分障害人格障害などの精神障害とAUDとの密接な関係は、精神障害がアルコールへの暴露のリスクを増加させ、AUDの早期生命リスク要因を悪化させる可能性があることを示唆している(Fink et al., 2016)。

統合失調症

  • 精神分裂病患者の3分の1以上がAUD診断基準を満たしており、人口全体の3倍以上の罹患率(Regier et al., 1990 ; Green and Brown, 2006)。
  • 統合失調症の患者における併存症AUDの重要な予測因子は男性、陰性症状の重症度、うつ病の重症度(Meszaros et al., 2011)、低学歴、以前の暴力的犯罪、物質使用障害の家族歴(Apantaku-Olajide et al., 2014)である。
  • 統合失調症患者のAUDの併存率は、精神病理のより大きな重症度(Margolese et al., 2004)および神経認知機能障害(Manning et al.,2009)と関連している。いくつかの研究は、統合失調症とAUDが併存する個人が、作業記憶(Bowie et al., 2005 ; Potvin et. al., 2008)、エピソード記憶(Smith et al.,2011)、および言語学習を含む記憶障害を悪化させることを示唆している(Manning et al., 2009)。
  • 認知機能障害、特に遂行機能障害は、AAUDを併存する統合失調症患者の主要な症状の一つである(Manning et al., 2009)。これは、遂行機能不全がAUDのリスク要因であることを示唆している。アルコール中毒以外の研究において、大麻関連障害のある遂行機能を有する人は、回復に成功するために必要なスキルを学び、適用することが困難であることが判明し、大麻使用への再発リスクが高まることが判明している(Crean et al., 2011)。同様に、これは、行動療法から恩恵を受ける個人の能力を妨げ、アルコール使用への再発のリスクを増加させる(Aharonovich et al., 2008)。

うつ病

  • アルコール使用障害およびMDDの併存症は高い(Boschloo et al.,2011)。一般集団における研究は、うつ病性障害を有する人々がAUDのリスクの2〜3倍の増加を示すことを示している(Hasin et al.,2007)。アルコール依存症と診断された被験者の12ヶ月の合併症に関して、29%の回答者が少なくとも1つの情動障害を有しており、最も一般的なものは大うつ病(28%)であった(Burns and Teesson, 2002)。MDDはAUDを誘発する病原因子である可能性が示唆されている。

パーソナリティ障害

  • パーソナリティ障害はアルコール依存症と併存する(Trull et al., 2000)。特にB群パーソナリティ障害と併存する(Mccarter et al.,2016)。
  • 外面化する病理(反社会的パーソナリティ障害[ASPD]と境界性パーソナリティ障害[BPD])とAUDに密接なつながりがある (Jahng et al., 2011)。BPDを有する精神医学入院患者の約50〜70%も、物質使用障害、通常はアルコール使用障害の診断基準を満たす(Zanarini et al., 2004)
  • ASPDのDSM基準を満たした患者は、ASPDをではない人々よりも、アルコール乱用および依存症を発症する可能性が21倍高い(Regier et al., 1990)。
  • 認知機能に関する研究は、パーソナリティ障害におけるアルコール探索が衝動性と関連する可能性があることを示唆している(Trull et al., 2000)。
  • 初期の衝動性の自己報告尺度は、B群パーソナリティ障害とアルコール依存症の併存例では、パーソナリティ障害なしのアルコール依存症よりも高かった(Dom et al. 2006)。
  • の衝動性は、将来のアルコール依存のリスクが高いことに関連しており、衝動性スコアの高い成人はアルコール依存症と診断される可能性が高い(Prince van Leeuwen et al, 2011)。衝動性とは、負の感情(Shin et al.,2015)、悪化した認知能力(Haaland et al., 2009)、および遂行機能の障害(Stevens et al., 2003)のこと。
  • 失業率、学校の成績不良、乱雑さ(Gregory et al., 2008)、神経学的負の感情(Trull et al., 2004)によりBPDとAUDに相関がなくなるとしている。

不安障害

  • 不安障害は、男性(19.2%)と比較して女性(30.5%)においてより高い生涯有病率を有する(Kessler et al., 1994)。メタアナリシスでは、不安障害とアルコール乱用(オッズ比1.636)とアルコール依存(オッズ比2.532)の間に強い関連性がある明らかになった(Lai et al., 2015)。
  • 様々なタイプの不安障害の中で、社交不安が比較的よく研究されている。National Co-morbidity Studyから得られた結果は、社交社会不安障害におけるアルコール依存症の生涯有病率が約24%であることを示している。この値は社交不安障害のない患者よりも10%多い。
  • Grantらは、社交不安障害の患者の13%、生涯にわたって社交不安障害の48%以上がAUDの基準を満たしているとしている(Grant et al., 2005)。
  • 社交不安障害を有する患者は、社交不安障害ではない人に比べリスクが2〜3倍高い(Morris et al., 2005)。一方、AUDを有する患者は、AUDを伴わない人に比べて社交不安障害を発症するリスクが10倍に増加する(Kessler et al, 1997)。
  • 外傷後ストレス障害(PTSDs)は一般有病率が3〜7%であり、PTSD患者のうち10〜61%がAUDである(Debell et al., 2014)。

自殺

  • 最近の韓国の研究では、AUDITスコアが20以上の場合、自殺念慮(オッズ比1.68)と自殺企図(オッズ比2.64)との有意な関連性が明らかにされている(Bae et al., 2015)
  • 1週間あたりの高頻度の飲酒(4回以上)が自殺企図と有意に関連している(オッズ比2.85)(Bae et al., 2015)。

双極性障害

  • 双極性障害の患者のAUDの障害有病率は46%である(Regier, 1990)
  • Merikangasら(2007)は、National Comorbidity Survey Replicationに含まれる9000人以上の患者から、アルコール乱用およびアルコール依存のそれぞれ39.1および23.2%生涯有病率を報告している。
  • 一般的な躁病、双極性II型障害(軽度の躁病、介入されたうつ病)は、生涯にわたるアルコール乱用およびアルコール依存症の強力な予測因子である(Merikangas et al., 2008)
  • ブラジルでは、双極性障害と診断された患者におけるAUDの有病率は約23%(Nery et al., 2014)。

青年期のアルコール摂取

  • 青年のアルコールに対する特定の脆弱性は、行動の抑制制御に関与する前頭前野などの特定の脳領域の成熟の遅れにある。
  • この前頭脳領域の成熟の遅延は、感情、状態調節、および恐怖に関与する扁桃体の活性化と関連していることが示されている(Gierski et al., 2014)。
  • 正常な青年期発達は、このバランスを逆転させて、扁桃体の減少とともに前頭皮質領域の高活性を回復させる(Gierski et al., 2006)。しかし、早期のアルコール消費は、アルコールを誤用する青少年が抑止課題および精神的柔軟性に乏しくなり、成熟プロセスを崩壊させる可能性がある(Winward et al., 2014)。
  • 若者の無茶呑み(Binge Drink)、若年中等度飲酒、禁酒の若い成人、健康な高齢者(平均年齢69歳)の4つの異なるグループの神経心理学的パフォーマンスを比較した。無茶呑み群は中等度の飲酒者または禁酒主体よりも劣っていた。高齢者と同程度のパフォーマンスであった(Petit et al., 2014)。これは若年の過度な飲酒が早期老化を引き起こしている可能性を示唆している。

青年期のアルコール使用障害の予後

  • 予想するほど明確なエビデンスは出ていない。
  • 青年期のアルコール消費が、成人における不安障害(オッズ比1.30)および反社会的人格障害(オッズ比1.36)の診断の確率を高めるという縦断的研究(Brooks et al., 1998)
  • McCambridgeら(2011)のレビューでは、10の異なるコホートからの35の報告と、個々のコホートからの19の他の報告を基に結果をまとめている。早期アルコール摂取とAUD以外の精神障害の予後の関連を調査したのは5つであった。これらの5つの研究は、抑うつ障害、不安障害、および自殺念慮/試みに特に焦点を当てていた。研究のほとんどは、青年期のアルコール消費量と成人(または若年成人)とうつ病や不安障害の診断の間に強い関連性を検証していた(Andreasson et al., 1991; Rohde et al., 2001; Wells et al. 2004; Mason et al., 2008)。
  • 2つの研究では、早期/思春期のアルコール消費量とうつ病性障害や不安障害との間の関連を確かめることはできなかった(Wells et al., 2004; Kandel et al., 1986)。
  • 思春期の問題(AUDの診断はないものの、1~2つのアルコール依存の症状がある)は反社会的パーソナリティ障害へ発展するリスクが高い(Rohde et al., 2001)ことが判明している。
